Pseudohyperphosphatemia in Multiple Myeloma

Abstract

Unexplained hyperphosphatemia was noted in a patient diagnosed as IgG-lambda type multiple myeloma. In this patient, the serum inorganic phosphate level was 2.3mmol/L(7.1㎎/dL) on the Technicon CHEM1TM. But the serum inorganic phosphate level was 1.4mmol/L(4.2㎎/dL) on the Hitachi 736TM and 1.5mmol/L(4.6㎎/dL) on CHEM1 TM after removal of the paraprotein by
ultrafiltration. These findings suggested that monoclonal proteins interfere in measurement of inorganic phosphate by colorimetric method. Our case demonstrated the
possibility that hyperphosphatemia in multiple myeloma may be due to the bindings of phosphate to monoclonal immunoglobulins.
